We are looking for Palliative Care Certified Nurse Practioners to perform home Palliative Care and Supportive care consultations.  Must be comfortable with taking care of chronically ill patients with complex diseases.  Responsibilities include symptom management, medication adjustment as needed, good communication skills, and ability to have difficult conversations if needed.  Most of our patients are still seeking curative therapy, and provider’s goal remains to help improve quality of life for our patients while working closely with patient’s existing primary care physician and other specialists.

Must have 5 years experience as an APN, with minimal 2 years in Supportive Care or Palliative Care.
